Hundreds of thousands of patients across Europe miss out on advanced medicines due to low biosimilar adoption, reveals Act4Biosimilars.

  • The Act4Biosimilars Deep Dive Report for Europe unveils opportunities to expand access to biologic medicines by identifying the key challenges to biosimilar adoption, building upon the existing issues of growing disparity between Eastern and Western Europe

Basel, November 16, 2023 – Act4Biosimilars, founded and sponsored by Sandoz, launches the Deep Dive Report for Europe, exposing the challenges preventing greater patient access to biosimilars in this region. The report encourages local stakeholders to use the Act4Biosimilars Action Plan to establish the strategies, tools, and activities needed to increase equitable patient access to biologic medicines and support healthcare system sustainability on a global scale. 

Zorana Maravic, CEO of Digestive Cancers Europe and Act4Biosimilars Steering Committee member said: “Biosimilars are a win-win situation for patients and healthcare systems alike. As a pioneer in the regulation and adoption of biosimilars, you would expect high and equitable levels of adoption across Europe. However, hundreds of thousands of patients each year are missing out on medicines with a proven track record of being effective and safe. This must improve, no patient should be left behind.” 

Although Europe has been a pioneer in the adoption of biosimilars, the Deep Dive report reveals differences country by country, including   disparities between Eastern and Western Europe.1 Additionally, within Western Europe adoption of biosimilars varies significantly between countries, and among disease areas. The report explores this, and the following insights: 

  • Anti-competitive tender processes threaten a healthy market for future biosimilar medicines.
  • Stark inequalities across the region, particularly in Eastern Europe. This is partly due to diverse reimbursement and procurement processes, signposting opportunities for cost savings and increased patient access. 
  • In many Eastern European countries, healthcare professionals are not provided with biosimilar educational materials in local languages which impacts their comfort with prescribing these medicines.

Biosimilars present an affordable, effective treatment option which have expanded access to advanced biologics, resulting in earlier, broader treatment- for patients. Since they were introduced in 2006, they have created over €30 billion in cumulative savings for European healthcare systems as of 2022.2 

Prof. Arnold Vulto, Independent Consultant and Educator, Vulto Pharma Education & Consultancy (VuPEC) and Act4Biosimilars Steering Committee member, said, “No healthcare system can afford not to use biosimilars. Following the publication of the report, the Steering Committee will be reaching out to local stakeholders across Europe to explore opportunities that support better adoption of biosimilars through implementation of the Action Plan. The Steering Committee will turn its focus to Middle East & Africa in Q1 2024, and Asia Pacific in Q2 2024.

Sandoz introduces Act4Biosimilars Action Plan to accelerate patient access to biosimilar medicines

  • Despite nearly 20 years of availability, biosimilars show worldwide adoption rate of only 14% in initiative-tracked countries, while reference medicines still represent 86% of biologic treatments1

Basel, June 15, 2023 — Sandoz, a global leader in generic and biosimilar medicines, today announced the launch of a global roadmap to increase patient access to biologic medicines. The Act4Biosimilars Action Plan is part of the Act4Biosimilars initiative, founded by Sandoz and launched in 2022, and aims to increase global biosimilar adoption by at least 30 percentage points in 30+ countries by 2030.

Professor Emeritus Tore K. Kvien, previous Head of Department of Rheumatology, Diakonhjemmet Hospital for 25 years and a member of the Act4Biosimilars Steering Committee, said, “The time to globalize biosimilars is now, so that advanced biologic medicines are accessible to patients who need them most. Their benefits are proven, and it’s time to bring them to more patients and health systems across the world.”

The Act4Biosimilars Action Plan highlights the most critical challenges preventing patient access to biosimilar medicines, as well as actionable steps to accelerate adoption by overcoming those challenges. The Action Plan will be complemented by a series of reports which provides analysis of the key challenges by region. The first region spotlighted is the Americas and includes the following insights:

  • In the United States, the interchangeability regulatory guidelines have caused confusion among patients and healthcare professionals.
  • Due to gaps in the regulatory pathways, patients in Colombia and Ecuador can be exposed to biocopies which may not meet the rigorous international guidelines provided by the World Health Organization (WHO) for the approval of biosimilars2,3.
  • In Canada, the U.S. and Brazil, educational materials exist, however a lack of alignment across stakeholder groups on the materials has led to confusion amongst healthcare professionals and patients.
  • Unsustainable procurement practices, such as single-winner tenders, are common in Mexico and Brazil, creating supply continuity risks and potential disruptions to patient care. 

To address these and other challenges, the Action Plan features 12 key initiatives to accelerate biosimilar adoption across the 4As – Approvability, Acceptability, Accessibility, and Affordability. It provides actionable steps designed to help local stakeholders foster a more favorable environment for biosimilars in their country and ultimately drive global adoption.

The Steering Committee is working with local stakeholders across the Americas to implement the Action Plan and identify and address the key challenges holding back wider biosimilar adoption. The Steering Committee will turn its focus to Europe in Q4 2023, the Middle East & Africa in Q1 2024, and Asia Pacific in Q2 2024.

For nearly two decades, the introduction of biosimilars have enabled expanded or earlier patient access to biologic treatment. It is estimated that between 2023 and 2027, biosimilars could generate $290 billion in savings globally4. However, biosimilars are still unavailable in most countries and there are significant challenges to expand access. As a result, their impact on patients varies widely.

Measuring Global Progress
The Act4Biosimilars Impact Index will measure and assess progress for 30 initiative-tracked countries in relation to the favorability of the local environment towards biosimilars, under each of the 4As. The Index will be updated every two years, to give local stakeholders shared visibility and understanding of the most critical issues preventing biosimilar adoption.

Act4Biosimilars aims to achieve a biosimilar adoption rate of 44% by 2030 across 30 countries in scope of tracking. This is a 30 percentage point increase on the baseline figure of 14% from 2022.1 These figures are based on comparing the volume of biosimilars against the reference medicine for each molecule for which a biosimilar is already available or expected to be made available by 2030.

Basel, May 31, 2022 - Sandoz, a global leader in generic and biosimilar medicines, today announced the launch of a new global initiative called ‘Act4Biosimilars’ to help address health inequity and inequality worldwide. Act4Biosimilars aims to increase patient access to advanced medicines by facilitating greater approvability, accessibility, acceptability and affordability (the 4 A’s) of biosimilars. The initiative is supported by a multidisciplinary Steering Committee of patient advocacy leaders, healthcare professionals, biosimilar experts and industry leaders from around the world, with a mission to increase global adoption of biosimilar medicines by at least 30% in 30+ countries by 2030.

A biosimilar is a successor to a biological medicine (also known as “reference medicine”) for which the patent has expired and exclusivity has been lost. Biosimilars match their respective reference medicine in terms of quality, safety and efficacy. Hence, the biosimilar can be expected to behave in the same way as the reference medicine in all indications and patient populations that the reference medicines are approved in. Biosimilars are used in the treatment and prevention of many disabling and life-threatening diseases such as cancer, psoriasis, arthritis, and diabetes. 

As a result of their affordability, biosimilars have opened up a new realm of possibility for patients by enabling biological medicines to become more widely available to those in need of these advanced, life-changing treatments,” said Zorana Maravic, CEO, Digestive Cancers Europe. “However, until we address existing biosimilar access inequities, far too many patients will continue to miss out on treatment with the best possible medicines.”

Act4Biosimilars.com outlines the 12 goals that have been identified under the 4As by the Steering Committee to achieve the mission. These include ensuring equitable pricing, involving patients in treatment decisions and streamlining biosimilar development. 

As an immediate next step, the Steering Committee is developing an Action Plan to provide the strategies, tools and activities needed to equip and empower stakeholders to realize these 12 goals, across the 30 countries and beyond. The Action Plan will be supported by Country Indicator Maps to track and measure the change that is being driven by groups, associations and organizations worldwide.

“While countries such as Norway are leading the way in biosimilar adoption and have successfully realized significant healthcare savings through increasing availability of biosimilars, there are other countries still progressing and aspiring to reach their full potential,” said Professor emeritus Tore K Kvien, previous Head of Department of Rheumatology, Diakonhjemmet Hospital for 25 years. “Misinformation on the safety, efficacy and science of biosimilars continues to cause confusion and impede uptake. With the Act4Biosimilars Action Plan, we will prioritize the steps needed to help better educate, inform and create action across all countries and regions.” 

The Steering Committee will co-create the Action Plan with a group of biosimilar experts and key stakeholders, including patient organizations, healthcare professionals, trade associations, think tanks, government bodies and professional societies.
